Dental, Vision, Medical and Mental Health Clinic PLEASE SIGN UP FOR A VOLUNTEER ASSIGNMENT TO SERVE AT THIS EVENT. ➡️ To be included in this event, an ASSIGNMENT MUST BE SELECTED. Signing up without selecting an assignment does not register you to serve. After signing up, please ensure you receive an ASSIGNMENT CONFIRMATION EMAIL. ❗ IMPORTANT If you DO NOT receive a confirmation email, you are NOT assigned If you do not receive an email, please contact me immediately at xflores@aomsocal.org. 📱 MOBILE SIGN-UP NOTICE If you are using a phone to sign up, please turn your phone sideways (landscape view) in order to see all available volunteer roles and assignments. Parking—carpooling is encouraged. ⏰ Arrival Time Please arrive by 7:00AM (unless otherwise arranged). Check in at the Volunteer Table for your name badge—badges must be worn all day. 🥪 Food Light breakfast and lunch will be provided. If you have dietary restrictions, please bring your own nourishment. 🎒 What to Bring Attire: Business casual (Clinical staff may wear scrubs) Water: Bring a reusable water bottle Storage: Limited – keep personal items to a minimum Clinical Staff (MD, DDS, RN, NP, PA, EMT, PharmD, Optometry, etc.): Please bring your own stethoscope and otoscope (if applicable). We’ll provide gloves, hand sanitizer, surgical masks, and optical equipment. 📄 Paperwork Reminder To help streamline check-in and avoid delays, please ensure all required paperwork is submitted before clinic day. To ensure a smooth and pleasant experience for everyone, we kindly ask that volunteers do not sign up on the day of the clinic. Thank you again for your commitment to serving our community.
